AZ V/JV Medication Forms

Hello Cougar Baseball Families, We have been informed that all JV and Varsity players need to fill out and turn in the medication form:


Please fill out and bring to Coach Lipka at practice tomorrow, If your player is bringing prescription medication they will need a doctor to fill out and sign the form.
